Below is the evaluation form for RISE mentors to complete. This form is designed to capture the primary strengths and weaknesses of your RISE intern and helps us assess the student quality of the program and guide these bright young students. Please feel free to share your evaluation with your intern. I will NOT share these evaluations with them unless you provide permission to do so. They will, however, receive generalized feedback from me based on my own assessment and comments provided by you.
